Filling out form 5563 Re Immigration Canada

For those who have filled out a CAIPS request form before, which numbers are these:

ATIP File number?

Requester's reference number?

Visa file number?

I only seem to have:

From Canada: my Client ID, my hubby's Client ID, a KIT ID, a Medical ID number and a B-file number

From Quebec: a file number and an individual reference number (off the selection certificate).

Do I even have the right form?

Visa file number should be the B file number. As for the rest, I didn't put anything, and I received my CAIPs just fine.

Atip should be blank. Requester is your client Id number and visa is the b file number.

Wow, I was lucky to receive mine then. :) Thanks for the tip about the requestor number.

Question, if I want to request my CAIPs again, do I need to re-submit the authority to release information form again? Or just the other one...?

I believe just the other one. You could send both just to be on the safe side but I am sure I read somewhere that you only need to send the permission form once.
