Name of background/criminal check forms required for cedula

Can anyone tell me the name/number exactly of the forms they now require to be submitted before issuing cedula. I have gone through all my paperwork several times but I seem unable to track down any specific name or number of the specific check required.

Go to your local police station in the UK and ask for the police check/records form. They will provide you with the forms that you need to complete and send off to the CRO office in the UK and you have to wait approx. 6 weeks for the response. You must give this original to the residency office in Santo Domingo, or to your lawyer in the DR. Ensure you keep a copy of it. The full addy is on the form where you have to send it to in the UK.

This is now a legal requirement in the DR for residency applications, from your home country, the Interpol checks are also still done though.
