I am confused with this form because in document checklist they ask completed form by sponsored and each family member , so my sisters , and my parents need completed and signed this Form? or your

I would very grateful for your help.

Hi Alis,

I believe you only need to submit 1 of these.

An example of when multiple forms would be filled out would be if you were sponsoring parents or more than 2 people over 18 on the same application.

In your situation, you are the applicant and bringing a dependant child so only 1 form listing all your family members is required.
