Was there maybe three months ago. The road isn't too bad, for the DR. A car will make it fine.

The entrance price, if I recall, was 150 pesos per person, and that includes the guide and the flashlights. The lighting that was installed in the cave was stolen (go figure), so you have to carry flashlights. If you have a very good flashlight, I would recommend taking it with you so that you can see the paintings better.

Walking in the cave is very, very excellent. There were a few places that had standing water, but that may not be the case given that it hasn't rained significantly in the area lately.

ONE IMPORTANT CAVEAT (no pun intended): make sure no one in your party is afraid of bats. There are lots and lots and LOTS of bats in some areas of the caves, and they fly very close.

And if you're up for a nice walk, on your way, stop by the Casa de Caoba, Trujillo's old palace. It isn't marked, so you'll have to ask.
