Both Hospiten and Centro Medico Punta Cana work with Universal. They're also basically your only 2 options. At either place if the doctor doesn't speak english, they can request assistance with the Public Relations personnel.
You need a copy of the "Care Providers" in Universal's group.It is a list of all their providers.
All the insurance companies have them, but sometimes you need to ask for one.
