"Is your drug dealing competition costing you money?" reads the form, which appeared on the department's Facebook page Tuesday. "We offer a free service to help you eliminate your drug competition!"

The form then asks readers to fill in several blanks, including the competing dealer's name, home address, phone number and place of business.
