Anyone have a template of what is required at the court to be certified for the father to take parental responsibility? This is not a custody document, it is a document that the father must have to get his childs passport from the UK when he and the mother we never married. Nothing at all to do with leaving the country without the mother and so do not confuse the two.

I am not American, I do not need a lawyer to change my pants and hold my hand every time I leave the front door. This is not a difficult undertaking, I was simply wondering if anyone had a template to save me time. Thanks for the advice, but not what I asked.
