Which doctor are you moving to ( Dr)?
If you meant the DR then you can't actually get a work visa per se and the thing you can get is a lot of work.
Assuming you will work for peanuts in a resort since they pay peanuts (if you are an elephant this is quite good though) then do what most people do and just come on a tourist card, overstay and pay the fine when you go after 6 months which will be about $10 US.
