The Medical System in the DR

Hi all, I have a couple of questions about the medical system in DR, I hope you guys can help:

1) I'd like to know if a "medicare" exists or do patients pay each visit to the doctor's office?
2) Are there any hospitals specifically for children in Santo Domingo or are the adult and children hospitals all in one building or center?
3) And my last question, which i might not be able to get an answer to is, who takes care of the ventilators (breathing machines) that are on patients in the intensive care unit (icu). Is it the doctor, the nurse or a specific person who's trained for this such as a Respiratory Therapist?

For doctors who are private, you either pay or you show evidence of insurance. There are a number of insurance plans available in the DR, with Segna being one of the main providers.
