You should contact the DR Consulate not CDN

The Canadian Consulate handles consular matters for Canadians, and visas to Canada for non-Canadians. The DR consulate would do the reverse. It's in Ottawa ( the one in Toronto is an honorary consul).

They have a web site in Spanish and English.

If you Search here there is quite a lot of info on the process.

Application forms available in the General Migration Direction

The governmental dept. competent to deal with residency applications of foreigners in the DR is the "Direcci?n General de Migraci?n" in Sto Dgo situated on Autopista 30 de Mayo (continuation of Malec?n) behind the theater "Agua y Luz" They give out these application forms and also information sheets telling you what you require to apply for provisional residency. Theoretically you should also be able to obtain such a form in the General Consulate of the DR in Toronto provided that they are not disorganized as it happens many times.

The one here in Toronto is a guy's house. Honorary only. Ottawa is a full service one and an ambassador.
