Your best choice is Verizon.

You should also read the hundreds of posts on this message board that relate to your questions. Many are in-depth and will make your life much easier when choosing phone or internet options.

The downside is it will require an hour of your time researching and reading

Depends where you live
In the cities and bigger villages you'll find DSL, outside those areas Dial-up is a possibility
If you cannot use any of those services satellite-internet is an option
