In an interview to Freddy Beras Goico about his recent illness and trips to the US to get medical treatment, Freddy said:
"In my family, the first priority is having international medical insurance (seguro internacional). When bills are due, we pay our international medical insurance bill first, and buy food second".

As far as I know, you're fine with a good Dominican seguro as long as you don't have some truly major problem, like cancer, broken spine and so forth.

Where do you buy "seguro internacional"? How much are you expected to pay? How "real" is it?
