You are talking about at least six hours by car with normal traffic. Taxi fares will vary depending on whether you are taking a tourist taxi or local cab. Also for such a distance, price may be negociable. I would guess about US$200.

San Pedro de Macorix does not have an airport, so flying is out.
Why not rent a car or take buses? You could also take a bus or taxi to the Puerto Plata Metro bus terminus. Then a bus to Santo Domingo, and then an express bus or taxi to San Pedro de M.

you could drive to Puerto Plata Taxi/Caribe Tour 1:15 minutes and then get Air Santo Domingo
Pop/LA- Punta Aguila International Airport in Casa de campo La Romana then A Taxi 30 minute to San Pedro De Macoris
