I have a son in the dominican republic and i did not include the fathers name on the birth certificate. I would now like to add the fathers name to the birth certificate and will be travelling to the country to do so soon. I would like to know what is required.

because there should be one.
an other scenario would be that she filled in 'unknown'.
in case that the mother of the child is in agreement with the inclusion of your name in the child's birth certificate just the 2 of you should show up together at the office where the certificate been issued and request the addition, would not be a big deal, no big $$ neither.
