Yes, it makes a difference. Taking an overnight flight from the West Coast through for example Miami/Charlotte will put you into to SDQ pretty much at the height of arrivals: busy time. So going overnight from the West Coast does not avoid the "2 hour wait in line". The only way around it we have ever found somewhat, is taking a day flight to the East Coast (NYC, Philadelphia, Newark) and then waiting around for the middle of the night flight. There are flights from NY that leave late at night or after midnight which would get to SDQ somewhat before the "rush". But that is a long day compared to 4 hrs. (approx) to Miami/Charlotte then 1.5 - 2 hrs. to SDQ. For many avoiding the "2 hr. wait" is not possible given where you are coming from. That is why some choose to use the Before Boarding option. Sorry but when I fly those flights I want to land and get out of the airport as quick as I can. Worth the price.
Yes, with beforeboarding, if you only have carry-on you will get out of the airport in 10 minutes. My problem is I travel like heavy and must wait for my bags.

Got Sky lounge last time through Business ticket, DISASTER after DISASTER, nothing to do with a lounge in any other country.

First of all very small, few uncomfortable seats. Food was very very limited with staff handing it out and drink options the worse.

Would skip it next time even if included.

I have had a couple of very nice experiences in there (although I think the manager kind of likes me, he calls me Dama Hermosa when he serves me and recognised me immediately the second time). The food was slow, I just had a little chicken ceasar salad, that's all I wanted, but he kept coming over and giving me another drink (four in the space of an hour I believe) and by the time I got to board my flight I was very merry indeed. I sat at a table, there was plenty of room when I went in. It's better than the previous Sala VIP that used to be in that location in my opinion, at least there is some proper food now.

Well depending on when your flight lands and where you are in the plane, if you only have carry on you will be out in 10 mins without beforeboarding. Apart from in the middle of the pandemic when they did that stupid testing, I have never waited in immigration in SDQ when arriving. I remember once there was a line but I flashed my residency and they pushed me through to the Dominican line where nobody was waiting.

I have never been that lucky. And I use the Dominican line.

Arriving not, apart from during the pandemic. But departing yes, I got stuck for over 2 hours a few weeks ago. (No tenemos sistema mi amol). My flights land at about 8pm usually, maybe that's a good time. I always fly business so I'm always one of the first off and quite often we walk into an empty immigration hall.

I've seen huge lines for arrivals in Punta Cana but skipped those with my residency card. So I guess I've been very lucky so far.

What time is "busy time" in your opinion?

Probably anything after 11:00am or 12 noon. Also, the weekday can affect just how crowded it can be. Now we sometimes use Santiago, stop at the in-laws and continue on to Santo Domingo the next day.
