This form is a disaster, especially in English. I couldn't get past the Date of Birth question. It kept bringing up a calendar and when I selected my DOB and pressed OK, it did NOTHING. Had to change to Spanish to finally get past the DOB.
Next section asks for 1. Port of Embarqement/flight #s 2. Shipping Port of Embarqement/flight #s and 3. Port of Disembarqement/flight #s.
I thought this would be pretty easy but it kept giving me error messages in each of the sections. I was only able to proceed when I put Otras/Other in the first section.....arrrggghhh!
Form took me nearly 30 minutes to complete.
