Visa application - visitor

I have exhausted every which way I know how to find a visitors visa application online.

Friend of mine lives in India. He wants to visit the DR with his wife and family. People from India must apply for a visitors visa.

So where the hell do you find a blank application online???? I can find instructions and prices but no actual application.

Thanks but what I want is a Visitors visa application form. That is all I need. I will email the embassy I guess. Just figured there MUST be one online for a regular visitors visa!
