Greetings!
(I post this because we live on the North Coast, however it does kinda applies to Santiago).

My wife has some problems with her knee. It’s been going on for several months now. We have seen a “rheumatologist” and had an MRI (HOMS STI). The rheumatologist noted that there was damage and put her on a regiment of “powdered chondroitin” along with a steroid shot directly into the knee for pain. Neither one of these things have worked. Not even a little. My thinking is that we need to find an orthopedic surgeon for further review. Can anyone make a recommendation as to a decent orthopedic surgeon? Preferably in STI at HOMS. MUCHAS GRACIAS!!!

You could try this guy as he speaks English well. There is a kneee specialist at Union Medica but I don't know if he speaks English or if that is a requirement for you.

Is she a possible knee-replacement candidate ? I feel for her. A lot of my joint pain went away when I went back to lifting weights, as strange as that seems. I think improved muscle strength give support to the joints.

If you see and orthopaedic surgeon the recommendation will most likely be surgery.
My understanding is that an x-ray will tell if you have bone/cartilage issues, and the
MRI should tell you if you have ligament issues amongst other things.

Assuming erosion of cartilage: Re-enforcing muscles around the knees is a good suggestion. Doing that will make recovery from knee replacement much easier. The exercise should not hurt. Before my double knee replacement I was biking 15-20 km per day - on flat, easy surface. I built up to that. That allowed me to delay replacement for 2 years.

CCCC...CC once gave me this advice: "why dont you go back to your own country where they have real doctors". Something to think about if it is an option.

..Bill you are so right...the gym is good for about everything....resent study, with a regiment of at least 3 days a week in the gym,and putting effort into doing things right. the t cell count of your immune system will sky rocket to about when you were in your 20's..also longer life span..in my case libido is way up,wonderful to feel like 30 again.........Doc............

Not sure about the libido. Maybe I'm not exercising the right muscle? But it sure helps the hips, knees and shoulders

In the gym where I go there is enough eye candy to let you know if your libido is still there. Actually I agree with the exercise pre-surgery and as a preventative delay. Many surgeon will encourage you to rehab rather than operate if there is a chance it will make a difference.

Dr. Miranda in Burnigal POP, operated me in Aug.2017 a total knee replacement.
Is not a light operation that I must tell honestly, but this man has done a great job, now walk as the best without pain.

Just another possibility. An American friend of mine loudly sang  the praises of Dr. Alfredo Guerrero Krantz in Santo Domingo. He replaced her hip. She was a medical professional in NYC, and not easy to please.

May I suggest you read the experiences of hundreds who put to practice the pain theory by Dr. John Sarno.

Your wife may be able to avoid surgery.

There is a webpage called: "Thank you, Dr. Sarno" that you can check out.

Unfortunately the steroid shot does not work for every case (been there). In the end I had surgery (torn meniscus repair and micro fractures done) but my surgeon told me that someday (sooner than later) I will need knee replacement surgery. I had my surgery done in the US for comfort/family reasons but had x-rays and MRI done here in Santo Domingo. It has been almost a year including 2 months of 4 day a week rehab. Trying to keep my other knee healthy by exercising, etc... Best to your wife.

I had knee surgery in Santo Domingo a few years ago. The doctor was excellent. Dr. Luis Gomez Infante at the Plaza de la Salud. He also works in La Vega a few days a week. Years later I have no scars and my knee is back to normal. Excellent doctor.
