If it's anything like the medical doctor who sees applicants for Canada... just call the doctor's office and they will tell you the price in both pesos and dollars (find their number on the website for the U.S. Consulate in Santo Domingo)! They probably only speak Spanish and the price for the medical for Dominicans coming to Canada was aprox. $150 U.S. - about two year ago.
