Certificate of good conduct

For residency, I need a certificate of good conduct. Is this a specific form or just a letter from the local police?

Certificate of Good Conduct. Issued in the DR only by the National Police (Av. Leopoldo Navarro) or the Attorney General?s office ?Procuradur?a General de la Rep?blica? at Centro de los Heroes. This document is valid for 30 days. Those under 18 years are exempt from obtaining this document.

I got mine at the police station on avenida Francia in Gasque. Cost 50 pesos plus 10 pesos for a sello (stamp).
