What are the rules regarding pronouncing x in a word? Sometimes I hear an h sound; other times its a "hard" x, as in axe. My grammar book doesnt cover that.

You hear the H sound in words like Mexico and a few names like Ximena. Mostly, X is pronounced like a J (the H sound) in names of Mexican cities. There was a change in the way this letter is pronounced. It used to have something like an Sh sound. It still does in Portuguese. Then it acquired the H sound and now it is pronounced like a regular X in most words. Mexico is still pronounced Mehico, or M?jico, as they spell it in Spain. In Mexico, the word Xoxo, is pronounced Jojo or Hoho. Centuries ago, Mexico was pronounced Meshico.

From my experience here in the DR the x in "nexo" sounds like a soft "nekso" or "eksamen" for "examen". If it starts with x it takes on the sound of the Dominican "z", for instance "Xiomara" sounds like "Ziomara". Also, as a side note, if a word starts with an "s" inevitably the people will pronounce it like "es..." (Esteven instead of Steven) but with an "x" or "z" they don't -as far as I have seen.

To my knowledge the word xoxo doesn't mean anything in the DR.
....and now that you mention it, I recently went into a perfume shop looking to buy perfume and the lady showed me a bottle that said xoxo. I asked the lady what the word meant and she said hugs & kisses.

Actually, the only time I saw XOXO, the name of town near Oaxaca, was during one of my trips to that city in Mexico. This is the way it is spelled, but it actually is a Zapotec word. The Spanish spelled it this way and this particular spelling continues today. It is pronounced HOHO.
