This is my expierience for taking my daughter out of DR on vacation to canada without her mother.
She has Canadian passport and has been here for more than 6 months and lives with me.

1 Notarized letter from non travelling parent. 2000 pesos

2 Birth certificate at the junta across from Margos
restaurant in sosua. 500

3 Pay at the banco reservas for the legalizing 700

4 get 2 copies of receipt 100

5 Take letter and receipte to the court house in Puerto plata 3rd
floor. They will tell you to come back in 1 or 2 hours and get it.

6 Online you can submit the application but I instead went to
a internet place and they did it for me and uploaded all the
documents. The letter, the birth certificate, copies of child
passport, mothers cedilla, my passport and fotos of the child (2x2).
Once he gets all the information uploaded he gave ma a
number to take to themigration office. 1000

7 went to the migration office, gave them the number
from the application, passport copies, original letter,
the 2 fotos and birth certificate. The fee is either paid
beforehand at the banco reservas or at their office
by credit card. 2000

total 6200 pesos all the headaches. PRICELESS

Now I have 3 months to use this and it is good for a single
use but when we return and it is within 6 months of the
return we can leave again and we do not need to do it all
over again.

The mother is in the process of giving me full custody and signed
by a court judge and in that case the process is the same but
without providing the mothers consent letter.

I've never heard of leaving within 6 months of return and not having to do the process again. Is that new?
My kids left 3 times in a span of three months and each time we needed the permiso.

So it's not for Dominican kids who are just visiting the other country.

Also new is that if both parents are traveling with the kid you don't need it.

I remember in the past when we have traveled together with our son, we had to get the same permission although we were both traveling with him. I found that really stupid.. But now that's gone.

And I would say that it's easier when you have the sole custody.

We just did it, we're leaving in a few weeks.

And it's just upload the required documents to their online service and then go to Puerto Plata and give them the papers and you'll receive the permission right away.

Birth certificate, 2 photos, passport copies, of both the child and the parent traveling with the child, and the custody paper. Easy peasy. However I don't know if they require you to go the court house every time to get it signed. Because we used the same custody papers that we used last time we traveled (6 years ago), but then they didn't need that signature on the last page.

No such thing as easy peasy in this country when dealing with government. From canada all you need is a notarized letter from the non travelling parent, like it was here before they introduced all these requirements.

Well it's an easier prodecure than having to get a Schengen Visa, so it's all relative what you compare it to. I guess in Canada you don't have any immediate issues with parents just "fleeing" the country with their kids to try to hide from one of the parents. I guess that's the reason why they have these measures in the first place. My wife told me about a big case here in the Puerto Plata area, don't remember who, was like 15-20 years ago. But apparently a famous "mobster" took his son from his mother and just left the country and nobody knows where they are now. They could leave because there was a few (or many) corrupt migration officers that although they didn't have the correct papers for the child, just let them leave anyway.
But I agree, it's still a bit annoying. But it's all relative to what other things you experience here (and in other countries). I mean wasn't for that my homecountry have become so increasingly affraid of foreigners that they more or less have created laws that stops people from coming into it (even those who actually have a legal right to be living in it) I would have left this country and live there a looong time ago. But that's life.... complicated.

I'll add my two cents:

Background : my daughter has a US passport ever since birth but had never left the country. She is 5 years old.

Travelled with my daughter (without Mom) for the first time last month May, from DR to the USA. We paid 7,000 pesos (all inclusive) to our lawyer in Sosua and received the permit the next day.
When we left DR, they kept the original permit at immigration (Santiago airport). We returned from the States one week later .

We travelled from DR to the US again last week (June) and our lawyer advised that another permit is not necessary if her passport is that of the US (citizen of US) as long as she hasn't been in DR more than six months
We left DR without any questions, just stamped and away ( Santiago airport again) . We are here in US still !

So basically one trip every five-six months out of DR and we wouldn't have to worry about permits !

Ok, I read that's only if they have legal residence/double nationality in another country and are travelling to that country.
Yes exactly, if the kids are Dominicans only without foreign residency, you have to get the permit every time..
