Can you tell me what public transport options (forget taxis) between la romana and San Pedro/Juan Dolio.

Similarly between Boca Chica and Juan Dolio/San Pedro.

Do Guagas run route, how regular, where you pick up transport etc

Assuming you are arriving at the La Romana airport, it's a little tough to completely take taxis out of the equation. Gua guas are not allowed to enter the airport property, and it's a bit of a hike to the highway. Not really feasable unless you are traveling very light. Your best bet is to get a taxi to the parada near the Parque Central in La Romana and catch a bus from there. San Pedro, Juan Dolio and Boca Chica are all on the same route.

Since I have a car, I really haven't had any reason to learn the bus schedule, but I know that when I have transported friends to the parada, they were not concerned about their arrival time. Between that and the steady stream of vehicles coming and going, I think it's safe to say that there are busses leaving in the direction of SD pretty frequently.
