I would stay on the north coast and choose different non all-inclusive hotels to stay as on the go. The recommendations are written in the suggested chronological order. I would rent a car for all these adventures. I would leave Samana for another trip.

Three nights staying in Cofresi or Puerto Plata:
Early morning... Damajagua Falls. Hike all the way to the top -- 27 falls, come sliding down.
Take time to visit Ocean World. Go in the late afternoon and stay for the night time show and disco time.
Next day: In the early morning, take the skylift up Isabel de Torres mountain and take time to walk the gardens at the top, now just where the sky lift drops you off. This is a true tropical botanical garden. In the afternoon, see Puerto Plata town: Visit Gregorio Luperon Museum and walk the downtown of Puerto Plata to see the Victorian-style wooden houses. Visit the Amber Museum. In the afternoon visit the San Felipe Fortress and take photos at the amphitheater park.If you trip coincides with a weekend show at the amphitheater park, you won the lottery.
Next day, take the Brugal Rum tour, take a stroll through nearby Playa Dorada to enjoy the golf course with its 50 year old plus tall trees and then a stopover at Playa Dorada beach.

Go west. Head out early. All day. Rent a car and head out very early to La Isabela. See the museum and ruins of the settlement built by Columbus. Tour the church built in La Isabela to honor the 500th anniversary of the arrival of the Spaniards.
Visit the La Isabela beach and have fresh fish on the beach.
Overnight in Punta Rucia. There are several hotels in the area. Arrange for taking a boat out to the Cayo Arena.
If you are into scuba diving, there is a apartment hotel run by a Swiss man with a good diving operation.

Return to the northcentral coastline. Book into a Sosua town hotel... enjoy Playa Alicia. Crystal clear beach water.
Visit the nearby Castillo Mundo King art museum built by a German who says he is in direct contact with extraterrestrials. The art in his museum helps you believe his story. Ask to see if you can visit the Jewish Museum. Visit Playa Sosua, snorkel out to the corals.
Two nights staying in Cabarete:
Cabarete Beach. Always something going on. Enjoy chilling out at the beach area. In the evening enjoy the beach parties also at the restaurant row on the beach.
Head out to El Choco National Park for a hiking and caving.

Love your recommendations: We will do most of this but stay based out of the resort we are booked at. Possibly one night out at a place in the west of Puerto Plata. Possible one night out to the east.
Question: entertainment at the amphitheater.
Only weekends?
How do I find out what is there on March 2-4 and 9 - 11?

You do not need to "book" anything to go out to Cayo Arena. There are dozens of boats that will take you out there.
I think I would just drive there and explore. For example:
Go West on the major highway, turn right towards Luperon and then watch for signs for El Estrecho. There you go to the left and follow signs to La Isabela. In La Isabela you go to the right and take the road to Estero Hondo. When you pass through Estero Hondo look for signs to La Ensenada, and go to the right. This road leads you right to the beach. Pay RD$100 to park. Go find ELSA, and there you can eat and rent a chaise lounge (?). Ask Elsa or her daughter in law about going over to Cayo Arena. Go early, and go in the middle of the week.
If you want, as an alternative, turn left as you get to the beach and go along until you get to the "village" of Punta Rucia. On your left there is a small restaurant called Damaris (ask for her). Good food, and she can set you up with a boat to the Cayo Arena.
Have a great time.
You might want to come to Santiago on a weekend to see the Carnival, a real special event.
