Suggestion......... Go to the office on Malecon after three months and pay the overstay fee. They give you the receipt etc. I gave this receipt to the felah at the airport after three years. He waved me on without checking the dates..
I wasn't going to say anything because of the naysayers who will claim that's not what they did five years ago, however thought I share my recent experience trying to pay my overstay and "legalize" my status at "La Direccion General de Migration (DGM)" Again is
my very recent experience. It may save someone some grief and certainly some time.
I for many years have payed my overstay fee as RAAAY has done. ( I use to get the extension just before my tourist card validity expired)- Easy day. In and out- 5~15 minutes. Why this way? It has given me a peace of mind knowing that my status in-country was legit once I went over 30 days. Just in case I accidentally ran over someone's chicken or whatever, the legal "system" would not be able to classify me as an "illegal alien".

Recently I went to pay, as I always done, Impuestos por Estadia (overstay fees) para Residencia Provisional
O Tarjeta de Turista (Tourist Card) at the Ventanilla Extranjeria (Foreigners Window). The line was about 30 deep (mostly Haitian nationals). After about an hour wait in line, the official stated I needed to complete an application and provide a medical certificate from one of their approved doctors. After some discussion that this can not be correct I left empty handed.
Two weeks later (and another long line) I tried again with a different official. After much discussion that this didn't make sense since I can, without formalities, pay the fee at the airport upon departure, I again left empty handed.
Third time a local paralegal tried. Same results, fill out application and provide medical certificate. ( I gave up, paid at the airport).
Here is the paperwork given to pay at DGM. Requirements:
1. Two copies of passport with remaining validity of at least six months.
2. Two copies of last DR entry stamp.
3. Completed form with current DR address.
4. Completed medical certificate from authorized medical doctor.
