You should have applied for permanent residency months ago. The new legal counsel at Immigration may not be as lenient as in the past when old provisional residencies were accepted toward obtaining permanent residency. (This legal counsel was the one who put in place the requirement that birth certificates for provisional residencies be authenticated at a Dominican Consulate abroad).
Expect at least to pay a fine.
