a travel book mentions J.A. Rent-a-Car and Estilo Rent-A-Car in Samana but gives no phone numbers.

You will probably have to wait until you arrive to make arrangements. They are located on the Malecon and usually have vehicles available. They are not cheap but it's a closed market. You might try contacting Caribe Tours. They are an excellent source of information. I don't have their number but they are located all over the country including Samana.
