Haitian passport renewal in Canada

Hi,

My husband needs his passport renewed (Haitian) we have tried contacting the Haitian embassy in Ottawa and Montreal to get the forms but they just don't reply. Is there a place to find the papers online? He has permanent residence in Canada - we need the new passport in order to apply for citizenship.
