Hmm... if that's the case, then the further back in time the cleaner all cities should had been. Dominican cities were often described as being very clean decades before Trujillo was even born. And this is what a US military guy said after visiting the DR and Haiti in 1913.

Historically the DR was the least dense of Caribbean territories, Puerto Rico always had at least twice as many people as the DR had (the DR surpassed PR in the 20th century), and Haiti tended to have upwards of 3 times as many people as the DR.
