Other than having someone pick them up in the capital, you might want to try one of the attorney offices in Sanitago that specialize in immigration.

Forms can be downloaded, I have done it myself. Go to the INS website and you will find all info there as well as forms. Otherwise you can also get them at the embassy in Santo Domingo.
