All depends on how it's being used. You can say, "Tengo ganas de comerme un helado" which means I feel like eating some icecream. It doesn't necessarily have to do with anything sexual...

As soon as I saw these words it reminded me of this beautiful song

By itself I would say : SEX

If used in a full sentence, as in: "Tengo ganas de ver un juego de pelota." or eat ice cream or whatever...

Then the meaning is I would like to

KateP ... defined it best

KateP has the best response and as mentioned in the thread it just depends on the context in which the phrase is used thus making the meaning clear.

It's an everyday phrase in Spanish....Tengo ganas de + phrase = I feel like.... which is the best equivalent/ translation in English IMO. An equivalent verb is apetecer.

Hillbilly has it right. If it's by itself it means just that. There's no other meaning in The DR for this phrase by itself other than "I want to F..." If in conjunction with other words then it will mean "I would like to..." do whatever it is that comes after which may or may not be sexual. Even though in a non-sexual way most people would use "Tengo deseo de...or Quiero..."
