I am in agreement with all of the above plus a couple of my own.
The bird cage is my largest peeve. Condo's who have thinking owners, and administrators who actually enforce bylaws, rather than break them to suit their own needs, and complain about the conduct of others!!!! Big gripe!
I have been on both sides of the medical drama, and the last being the best!! HOMS in Santiago and the Doctor I saw was one of the most professional and thorough I have seen, including Canada. Insurance was also helpful there. Big surprise!! but a pleasant one!
Drama is perhaps the best word, a lot of things here turn into a "DRAMA", small world, small life, small minds????? not sure, but it can get loud and aggressive in a hurry!!!
These things are just my pet peeves, and of course, only my somewhat humble opinion!!! Take what you like and leave the rest or of course the country!!

i would like to have silence sound system. you know, just like dominicans have ruido sound systems my machine would spread silence at the same distance dominicans spread noise.
and automatic immediate burn of third degrees on every hand that throws rubbish on the street.

and to para phraze one of the caesars if all motoconchos had only but one head... and i had a macheta...

Doesn't matter where you are living in the country, this is about the ONE thing that you would change above all about where you live that would make life so much better for you only, or everyone, or even the wildlife, doesn't matter. If a wish was to be granted, what would you get, or change. This can not include masses, like the entire police force or government.

If they would make the subway go from the Airport into central my life would be complete. If it could be extended so we could visit the beach without drink driving then that would be even better.
My wish!!

The areas from the City tot he Airport and Boca Chica are not that heavily developed, I would think a "Tram" or light rail system would be much less expensive with many more stops at much less cost

Or possibly run sections of the current Metro above ground such as many cities currently do, no need to tunnel and build expensive underground stations in that open area

raw beauty

What Frank the Tank said.

I agree with rice&beans. Better, drinkable water would be nice... For me- for the sole purpose of having more coffee shops around. I love coffee, but the water situation diminishes my choices drastically.

Other than that.... me thinks that the D.R. has a certain 'raw beauty'... that may forever be gone if it gets too refined. Or maybe that's OK if it did. I worked in Florida for 6months.... It wouldn't be the end of the world if the D.R. got to be like the Miami suburbs area.

Chirmoya: "...Naturally, when you don't have insurance it's a lot smoother. They're practically falling over themselves to treat you...."

None so blind as those raised in a socialist system ? you've pegged with your own words what's RIGHT about the DR and WRONG about socially engineeered systems elsewhere, but I don't think you know yourself that you said it.

Try using the system here on its own terms, not your home-learned ones. You'll find it works better for you that way.

I will admit, however, that DR gummint thugs have discovered they can mimic the big boy countries with insurance Ponzi schemes ("it's for the children") and skim the cream while it lasts. That is causing escalation in fees across the board, which means the system that worked so well for me in the past probably won't outlast me.

However, I don't feel sorry for those coming behind me. They brought it on themselves by expecting a free lunch. Later they'll expect to get paid to eat it ? but with the inflated proceeds of their labor.

PS on 'order de llegada' vs. NHS etc. 'appointments': First a Dr. visit to get a prescription for tests, then another appointment for the labs, then another appointment for the Dr's diagnosis visit ? can take weeks. Here you get drugs and labs without prescriptions ? unless you've compromised yourself with insurance's promise of a free lunch ? and Dr. visit all in one day, max two.

Next step of course, they'll force you into their Ponzi scheme by tying drugs and labs to Drs' prescriptions.

you must be a doctor here or else nuts :)
health system here is totally messed up and i too prefer normal appoinments. even in europe you can do visits and tests all in a day - if you pay, that is. in DR you have to wait your turn regardless whether you pay or not.

Really pelaut, this thread was set up for people to mention in passing one thing that they personally would change so it is subjective. Many of the things other posters here listed don't bother me as much but it's interesting to hear their point of view.

I've been in situations here without insurance where I've had to pay, and naturally the process has been much swifter - less bureaucracy involved and the promise of all the money in one go as opposed to having to claim it back from the insurance companies is most attractive. I can see this from their point of view but neither should they or the insurance companies abuse patients with insurance - their health needs are just as urgent.

Yes, I'm the product of countries with state health systems and was always happy to pay the taxes and national insurance contributions that make it possible. I'm also the daughter of a doctor who went into medicine purely out of vocation and "could have made a mint in private practice" - quote from my mother ;) - but he didn't on principle. So I can't help it if some of his values have rubbed off on me. In fact I'm proud of it.

By the way, we're now in the second week of arguing with the insurance over the approval of the "aesthetic" eyelid growth removal.

General Practitioner

Everyone's experience is different and of course my anecdotes don't amount to hard evidence. I can only conclude that I must have been inordinately lucky in practically ALL my experiences with the NHS and cursed to the depths of hell in practically ALL my experiences with the DR private health system. :D

In the UK they would give you a GP's appointment, say for 9:15, and you'd wait no more than 10 minutes beyond that time, which means you'd only miss part of the morning at work. Here you have to arrive extra early to be first or one of the first on the list, and when the doctor finally swans in at 11 or so - providing s/he doesn't have an "emergency", the queue starts.

I also much prefer seeing general practitioners as they can deal with most issues; here you have to waste time playing "find the specialist".

Naturally, when you don't have insurance it's a lot smoother. They're practically falling over themselves to treat you.

A General Practitioner and English speaking would do very well here. I also have never understod why you have to go directly to a specialist. You should only go to a specialist through a referral from a general practitioner.

Also I see that pelaut is arguing that the system works well here if you're prepared to live without insurance and pay as you go, which seems to work for him.

For us it would be a risky strategy - if we could be guaranteed that we would only needed routine check-ups and minor interventions we would save money and frustration. It would almost certainly come to less than the US$1,500 or so per year that we spend on insurance. But if these things could be guaranteed the insurance companies would go out of business.

I dont have insurance. For number of years also while living in Cuba before I had just coverage for catastrophic from an international insurance company. I am considering getting full coverage because I find that when you say you dont have insurance they see an oppurtunity to make things up and it is hard to talk to a doctor who does not speak a language that you command fluently. One of the hardest things in a foreign country is going to a doctor. And here they are not trained in explaining anything to the patient. It is just this is what you have and this is what you have to do.

and because they are such a little backwards island where folks dream only of leaving...they do not experiment with food.. and they ruin their health with a mediocre diet .. heavy on startch and sugar.. so that diabetes is rampant.. despite the fact that there are some of the best veggies in the world here.

WELL, I thought the same thing(BUT A DAY BEFORE YOU DID, haha), but I was just going to write, it would be great if people could contemplate the usefulness of personal responsibility.
a. This whole "it is not my fault thing" I dont like.
b. This waiting for "rescue or resolutions" I dont like.
c. The compliance with purposely lying while predicating that the other person doesn't know what a lie is.
d. When meeting a foreigner it would be great if you were not always looking for the Big Score. You can slow roast a chicken and enjoy it more.
e. If people could speak simply, concisely or in absolutes and not in ephemerals, euphemisms or riddles.
f. Likewise, if they really knew what a real friend was, what prostitution really is, where your problems really lie, why you really cant use the language like other Spanish speaking countries.
g. I am biting your style, Annie: The Garbage. You have to understand people will not return to see "paradise" if they see trash all over the highways. This is the same as d. Once the tourist arrives, you should be selling the return visit. Establish a money train that does not rely on Whores & Western Union(sounds like a good cowboy movie). You expect the trash somewhat in the big cities but enroute to tourist places, people shouldnt be looking out the window of their car or bus and see more garbage on the side of the road than flowers. I suspect this is one of them "water wells run dry" parables.

h. I wouldnt blame the Diaspora though. Maybe for lying, but I think it is more liking the first verse in the Michael Franks song "Blue Pacific"

Here I am: "Hometown Boy Makes Good"
Writes one critic.
All my childhood I lived
By the blue Pacific.
Anytime that reality
Makes me heartsick
I come here just to stare
At the blue Pacific

See? They just trying to show to the people back home that they have made it. They are a success. We can discount how many of them have to concentrate themselves primarily in one city or how they are eaten alive by poverty and the criminal justice system. Or how they are tracked into one low paying occupation or another or how they are obligated to send their children to underperforming schools. I was in the Bronx on Sunday and there was a big whoop about how some young girl last name Bautista, 12 years old, got arrested by the NYPD for getting into a fight in school. Now her mom cant speak english and is getting intimidated by the police as the whereabouts of her child. Does anyone else see this? With one fell swoop, this child has been steered from the principals office of a low quality school district to the criminal justice system via family courts and subsequent probation for what used to be childish indiscretions, fighting in school.

i. I personally...personally, dont have a problem with man that wants as many girlfriends as he can have. My sole problem with the dudes is to be in the mix making children they dont care about nor want. Just as a claim toward supporting their virility and machoness. I dont like that.

But on the same accord I dont give women a pass for refusing to let some of these men see their children. For becoming shrews when the relationship is not going as you fantasized or gossiped to your other female friends. For laying with the guy who suddenly became a no-good-nik AFTER you got pregnant.
